GLASS Application Process

Required forms and waivers.

The following are required to be accepted for this program:

ALL seven participation forms, completed and signed (see below)
Recent photograph of yourself (see below)
Essay in which you tell us:
- Why you want to participate in this program
- How it fits into your academic or life/career goals
Letter of support from a faculty member
Meeting with the Undergraduate Study Abroad Office
- Contact Claire Caroll (ccarrol3@lesley.edu, 617.349.8323) to schedule a meeting
Medical requirements:
- Proof of consultation with a travel clinic and receipt of yellow fever and other immunizations currently recommended for travel to Guyana
- Have an anti-malarial drug prescribed and bring these with you on the trip

Participation Forms

Each file will open in its own window or browser tab when clicked, so simply close the document after printing to return to this page. If you prefer, right-click a link and choose the Save Link As option (or something similar) to get a copy of the PDF on your computer's hard disk.

We also need to have your photograph, on an 8 ½ x 11" sheet, on file in case of an emergency. This should be a picture with a clear and easily recognizable view of your head and face. A regular photo you have taken is fine and it can be of any size. It is preferable if the photograph is a picture of you alone, but one other person in the photo is okay as long as you make clear which person is you.
