Student Health Service Forms

All students must complete and return all forms prior to arrival on campus. Deadline for return is August 1 for students beginning study in the fall and January 1 for students beginning in January. All information disclosed will be kept confidential and will be shared only with appropriate University personnel on a need-to-know basis.

SHS Events
  • The annual Lesley University Student Health Fair will be held February 14, at the Student Center from 11-2.
After Hours Emergencies
  • Urgent care and all medical emergencies will be cared for by local hospital emergency rooms and walk-in clinics.
  • In the event of an emergency while on campus, students should contact their Community Advisor or Campus Security (617.349.8888 or 617.491.4444).
  • In life-threatening situations, students should call 911 for an ambulance, then call Campus Security so they can assist the ambulance in finding the student. Campus Security also has a first aid kit and defibrillators available.
