YOUR FIRST APPOINTMENT AT THE COUNSELING CENTER

APPOINTMENT, CLIENT INFORMATION FORM & CLIENT CONSENT FORM

The services of the Counseling Center are offered free of charge to all matriculated students who take classes on the Cambridge Campus or at AIB.

Students may make an appointment by:
  • coming in person to the Center at Lesley, 23 Mellen Street, 3rd floor, in Cambridge or to the Office at AIB, 700 Beacon Street, Room 301, in Boston
  • calling the Cambridge Center at 617.349.8545
  • calling the Boston Office at 617.585.6745

We request that students read and fill out THE CLIENT INFORMATION FORM and THE CLIENT CONSENT FORM BEFORE their first appointment. Your counselor will go over it with you, and answer any questions or concerns you might have, and ask you to sign it.  This is an important part of the counseling process and helps us to provide the most appropriate service to you.

For your convenience, you may also print out the client information form and the client consent form
from this web site or you can pick the forms up at either counseling location.
