Transcript requests

In order to be sure you receive the type of transcript you need in a timely manner, please choose your transcript type from the links below. Be sure to read the transcript request guidelines carefully.

Official transcripts

All students (except AIB students who graduated before 1990) may choose one of the following options:

  1. Submit a completed and signed transcript request form or a requirements-completed transcript request form with $5 for each official transcript requested.

    OR
  2. Submit a written and signed request including the following information, clearly printed: Name while attending, dates of attendance, social security number or student identification number, graduation date, signature, $5 fee for each transcript requested.

    OR
  3. Visit the Office of the University Registrar (1815 Massachusetts Avenue, 3rd floor), during business hours, with proper identification, and $5 fee for each official transcript. The standard 3 - 5 business day processing time applies.

    OR
  4. Log in to your LOIS account and click on Official Transcript Request. Requirements-completed transcripts cannot be ordered through LOIS.

AIB students who graduated prior to 1990 should submit requests as above, but to the following address:

Requirements-completed transcripts

A requirements-completed transcript is an official transcript that includes the completion date of the student's program requirements (generally the date the final grade for the program was received by the Office of the University Registrar), and the anticipated conferral date of the student's degree. This type of transcript is offered so students can prove they have completed their program prior to degree conferral (graduation). It is often used (for example) by teachers, who may receive a pay raise upon completion of their Master's degree requirements, or by undergraduate students applying for initial educator licensure. This type of transcript cannot be ordered through LOIS.

Note: Students must file a declaration of intent to graduate online before a requirements-completed transcript will be processed.

Endorsed transcripts for educator licensure

Students who have recently completed an educator licensure program, and who have not yet applied for, or are in the process of applying for, their educator license must request an endorsed transcript from the Lesley Certification Office. Please review the "apply for a license" page on the Certification Office website and then electronically submit the endorsed transcript request online form found at the Certification Office website.

Please note that bachelor's degree students who are applying for licensure must prove they have completed their program. These students must have either received their diploma or must submit a declaration of intent to graduate to enable the creation of an endorsed requirements-completed transcript.

Notes:

  • Make checks payable to Lesley University.
  • Telephone requests are not accepted.
  • Transcripts are not issued for students having financial indebtedness to the University.
  • Please allow 3 - 5 business days, plus mailing time, for processing requests.
