Financial Aid forms for graduate Students

On Campus / Distance Learning Students: If you will be taking classes online or on campus in Cambridge, MA, you must submit the FAFSA form and a Graduate/ALD Institutional Financial Aid Application.

Collaborative School Students: Please complete a FAFSA and, if you are beginning your program summer 2008 or later, a Collaborative School Financial Aid Application.

Cohort Students: Please complete only the FAFSA, unless we have sent you a letter asking for other documents.

• FAFSA Application Form - Don't forget to include Lesley University's school code (002160) so that our office can receive it electronically.
  • For the 2008-2009 Academic Year (September 2008 - August 2009), please complete a 2008-2009 FAFSA.
  • For the 2007-2008 Academic Year (September 2007 - August 2008), please complete a 2007-2008 FAFSA.

Loan Increase Request Form

If you have already been awarded a Federal Stafford Loan, click below if you would like to request additional Stafford loan funds.
