Dates and Deadlines for Health Insurance

Important Fall Dates

Students may waive or accept health insurance through their LOIS account for the fall semester from July 16th through September 15th. After September 15th, the online waiver will no longer be available.

After the September 15th deadline students must petition to waive or add health insurance directly through Gallagher Koster Insurance at www.gallagherkoster.com. Petitions are available from September 16th through October 4th. After October 4th, students may no longer waive their health insurance enrollment.

The 2011-2012 annual health insurance rates are as follows:

• Undergraduate: $1,231.00
• Graduate: $1,802.00

Students who waive or accept health insurance for the fall semester will not need to repeat the process for the spring semester.

Important Spring Dates

Students must waive or accept health insurance through their LOIS account for the spring semester from December 16th through January 15th. If the health insurance is not waived or paid for by January 15th, a late fee will be assessed to the student account.

The 2012 spring semester health insurance rates are as follows:

• Undergraduate: $835.00
• Graduate: $1,235.00

The due date for Spring 2012 payments, including the health insurance, is January 15th. Accounts paid after the January 15th deadline are subject to late fees and student account holds. The deadline to waive student health insurance through LOIS is February 3rd. After the February 3rd deadline, students must petition to waive or add health insurance directly through Gallagher Koster at www.gallagherkoster.com. Petitions are available from February 4th–February 17th. After February 17th, no changes to health insurance enrollment will be allowed.
