Program Change

Within A School - Request For

Students wishing to transfer from one program to another may obtain the Request for Program Change Form from the Registrar's Office or the program office in person, or by mail. This form must be completed, with all requested signatures received, and returned to the Registrar's Office, if the student has been officially accepted and currently enrolled. If the student had not yet been officially accepted, they can return the form to the Office of Admissions for Graduate Programs or Lesley College Admissions. In all instances, the student requesting a transfer from one program within a school to another program in the same school should consult with the academic advisor or program advisor of both programs prior to completion of the program change form. Students may be required to submit additional materials to the Office of Admissions for Graduate Programs or for the School of Integrative and Experiential Studies for faculty review. A copy of the new program of study must be attached to the program change form.

Transfer From School To School Within Lesley - Request For

If an enrolled student wishes to transfer from one school within Lesley to another, the student may obtain the Request for a Change of School Form from the Registrar's Office or the program office in person, or by mail. In all instances, the student requesting a change from a program within one school to a program within another school must consult with the faculty advisor or program advisor of both programs prior to completing the Change of School Form. The student meets with the appropriate advisor for the program to which he or she wishes to transfer and learns of any additional requirements that must be met to complete a transfer. The student then must obtain the signature from the current advisor, indicating good standing in the current program. The student then completes any additional work required by the program in the new school. When the student has completed any required tasks to the satisfaction of the program director in the new school, the program director signs the Change of School Form and submits it to the Registrar along with copies of any additional work that was required and the new program of study.
