Application and Documentation
Individuals with special needs may apply for disability parking. These individual must fill out the Special Needs Parking Application stating the reason for special consideration. Physician documentation supporting the request must be attached. Individuals with a State-issued placard will need to verify that the permit has been issued in the same name.
