Personal Information Changes

(effective 1/1/07)

If you have any change relating to personal information that would be contained in your employment record, such as your name, address, phone number, or marital status, please visit the Human Resources website. Inaccurate information may lead to problems such as misdirected checks or W-2s, or delays in contacting you, or your family, in case of an emergency. How to make changes to your personal information:

• Log into My Lesley
• At the welcome screen click on "my account preferences"
• Under the Personal Information Section, click on "Edit Personal Information"
• Click on "Click here to update your personal information"
• Make changes
• Submit the form
