Purchase Order Policy

Registrations that are paid by purchase order must include on the registration form the purchase order number, the name of the conference, the name of the registrant, and the name of the issuing school system or agency. The original purchase order with authorized signature must be included with the registration form(s). Please retain a copy of the purchase order for your records.

Confirmation Letters

A confirmation letter will be sent prior to the Workshop. Please use an address or email address where you are confident you will receive your confirmation letter in a timely manner. If you do not receive a confirmation letter within 30 days of mailing your materials, please call to verify your registration. Confirmation letters will not be sent after Monday, July 16, 2007. If you register after this date your confirmation packet will be available at the registration desk at the Workshop.

Check-In Procedure

Upon arrival at the Workshop participants are asked to check in at the registration table to pick up their Institute Welcome Packet.

Cancellation/Refund Policy

Refund requests received before July 11, 2007 will be honored with a $20.00 administrative processing fee deducted from the refund. Requests must be made in writing via email to Heather Fox, at hfox@lesley.edu. Refund requests received after July 11, 2007 cannot be honored. Registrations received after July 11, 2007, are non-refundable. The refund policy is the same for prepaid and purchase order registrations. If at the last minute you are unable to attend, we suggest you send a colleague in your place.

Workshop Evaluations

At the end of the Workshop, please take time to complete the Workshop Evaluation Forms that will be provided to you. After filling out and turning in your evaluation forms you will be issued your Certificate of Attendance.
